Minimum of 2 years of nursing experience

**Job Summary
**
The Primary Care Nurse is a clinically competent practitioner of pediatric, family-centered nursing care.
This individual is responsible for providing safe, high-quality patient care while collaborating with patients, families, providers, and interdisciplinary team members to ensure positive outcomes.
Key responsibilities include:
Utilizing the nursing process through assessment, planning, implementation, and evaluation.
Managing care for patients with moderate to complex healthcare needs.
Collaborating with care teams to develop and update patient care plans.
Providing patient and family education and coordinating care needs.
Maintaining accurate and timely clinical documentation.
Complying with all professional, regulatory, and organizational standards.
Participating in patient care rounds and practice improvement initiatives.
Demonstrating professionalism and a commitment to delivering an exceptional patient experience.
Rotating to other Care Network sites within the designated cluster, as needed.

**Required Skills & Experience
**
Basic computer proficiency, including use of common hardware and peripherals.
Working knowledge of Microsoft Office applications (Word, Excel, PowerPoint, Teams/Skype).
Ability to access and review patient information within clinical information systems.
Experience utilizing clinical data and quality improvement tools.
Knowledge of confidentiality, privacy, and information security requirements.
Strong clinical judgment, communication, and organizational skills.
Experience caring for patients with moderate to complex health conditions.
