Job Duties
- Responsible for on-site or community supervision, problem solving and advocacy on behalf of program participants
- Assist in creating a safe and enriching environment for person's served in CW's Rehab division program by planning for and providing activities that are of interest to and within the capabilities of those served.
- When working in an employment program will enhance participant's opportunity for success; coach them toward appropriate work behaviors, efficiencies, quality and independence.
- Transport clients to job sites and/or community activities.
- Follow any established behavior, dining or other plans and document client progress and any other required data.
- Participates in required training sessions and maintains certifications, which include First Aid and CPR, PMT, Bloodborne Pathogen Training, Public Service License (based upon position). Employees with medical conditions, which prevent active participation in or prohibits certification in any of these required trainings must discuss the matter with their supervisor and submit a request for a waiver.
- Assists the Rehabilitation Counselor in developing Individualized Plans by providing information on program participant's progress, outcomes and involvement.
- Serves as CW's program participant liaison and advocate to employers, DDS staff persons, family members and/or residential providers. May attend team meetings.
- Upholds confidentiality in accordance with CARF, State and Federal Agencies (DDS, DHMAS, DCF and etc.) and CW employees.
- Employee will provide support to persons with disabilities in a variety of activities - for example: Community outings, job coaching, personal care may be required.
- Trainings will be provided for the various sites and the individual will have set hours but assignments will vary from day to day - must be open to accepting a variety of possible roles on a daily basis.
- Driving company vehicles will also be required
Essential Duties
- Able to travel from one location to another and effectively adjust to various work and program locations.
- Will not have a set routine - must be flexible to varying daily assignments that may be delegated the same day.

Qualifications and Education Requirements
- College Degree preferred
- At least three (3) years in the human services field

Physical Demands
The position can be a physically demanding position. While performing the duties of this job, the incumbent is regularly required to reach using hands or arms. The incumbent must spend the entire time at work standing, walking, lifting heavy objects etc. The incumbent must frequently lift and/or move up to 50 pounds and occasionally lift and/or more than 70 pounds. Specific vision abilities required by this job include close vision, and the ability to adjust focus. It is important to note that the Day Support Options Specialist may have to transfer individuals from chairs to wheelchairs.

Background Screening
Must submit to background check prior to hire and ongoing as dictated by contract. Must be able to pass criminal and drug screening processes to meet current contract eligibility requirements that apply.
